Prior to employment all applicants must submit to the following:
License Verification
11 Panel Urine Drug Screen
Physical
PPD Skin Test
Hepatitis B Series/Declination
State and Federal Background Check/ WV Cares Fitness Determination
Vaccination Records and/ or Titers (Facility Specific Requirement)
Interview Including Three Professional References
TRAINING & CERTIFICATIONS
All clinical staff of LHI must complete annual training and recertification. All training requirements are in compliance with OSHA regulations and must be completed prior to employment and updated on an annual basis. The following training modules are mandated for clinical staff by LHI.
